This is a space booking request form, all requests must go through an approval process and are subject to space availability as per our EastRidge Facility Use Policy. Please note that the approval process can take up to 4 weeks. To help us assess your space needs in a timely manner, please complete this form with as much details as possible.

Additional Insurance
Please note that all our bookings require a copy of certificate of insurance that is not included in our rental fees. All rental groups must provide EastRidge a certificate of insurance for not less than $2,000,000 showing EastRidge Church as an additional named insured. Insurance can be purchased from third-party brokers such as Duuo Insurance.
